Man Charged with Murder After Stabbing Attack in Merrylands

A man has been charged with murder following a brutal stabbing incident in Merrylands, a suburb located approximately 30 kilometers from Sydney’s city center. Emergency services responded to reports of the attack at around 10:00 on February 17, 2024, which resulted in one death and multiple critical injuries.

Setefano Mooniai Leaaetoa, 25, faces serious charges, including murder and two counts of causing grievous bodily harm with intent to murder. Authorities allege that Leaaetoa, who reportedly absconded from a mental health facility just ten days prior to the attack, attacked strangers in broad daylight. He allegedly escaped while being transferred between hospitals, raising significant concerns regarding patient security protocols.

Emergency responders arrived at the scene shortly after the incident began, which is believed to have started inside a Nepalese grocery store. One victim died at the scene, while a 21-year-old remains in hospital in critical but stable condition. A 47-year-old man is also hospitalized in stable condition, according to the Western Sydney Local Health District.

Witnesses described a chaotic scene as Leaaetoa allegedly fled the store on foot before being apprehended by police on nearby Smythe Street. During the attack, a knife measuring approximately 25 centimeters was reportedly used, as confirmed by Superintendent Simon Glasser.

Leaaetoa was known to law enforcement prior to this incident due to a history of petty crimes and mental health-related episodes. His case is complicated further by ongoing legal issues, including a separate charge for domestic violence-related assault.

Investigation into Mental Health Protocols

Following the attack, Rose Jackson, the New South Wales Minister for Mental Health, announced that a review would be conducted to understand how Leaaetoa managed to escape from a team of health professionals while being transferred to Westmead Hospital. Jackson stated, “All of the proper protocols were followed in relation to that transfer, but we have initiated a full review of that and will reflect on any learnings.”

Leaaetoa had been a recent patient at Cumberland Hospital, which specializes in acute inpatient care for individuals with complex mental health needs. The health district confirmed that he absconded on February 7 while being transferred for further medical assessment. Police and family members were notified at the time, but efforts to locate him were unsuccessful.

As the legal proceedings unfold, Leaaetoa is scheduled to appear before Parramatta Local Court again on May 13, 2024. His legal representation, provided by Legal Aid, did not request bail during the initial hearing.

This incident has raised serious questions about the safety measures provided in mental health care institutions, prompting calls for comprehensive reviews and potential reforms to ensure the safety of both patients and the public.
